Specialized Compassionate Care
Healdsburg District Hospital’s Subacute Unit provides care to residents who no longer require acute hospital care but need specialized care. These residents have complex needs beyond those of a typical skilled nursing facility. Subacute patients are medically fragile and require special services, such as inhalation therapy, tracheotomy care, intravenous tube feeding, and complex wound management care.
Our dedicated and compassionate team offers treatment options provided by nursing staff, physical, occupational, respiratory and speech therapists, social workers, activity directors, and dieticians, along with consulting physicians.
On-site Medical Services
Patient care is provided in a safe and loving environment with on-site medical services, including:
- Radiology
- Surgery
- Intensive care
- Pharmacy
- Occupational and physical therapy
For regular evaluations, we are able to provide care for the patient in-house.
After patient assessment, which includes input and conversation with family members, our Activities Coordinator will tailor events to meet the preferences of each patient. Our social worker and Subacute Unit manager are available to discuss plan of care and address questions. Families and friends are welcome and encouraged to visit patients!
All persons entering a subacute facility must do so by order of their physician or by referral from a current care hospital.
